You have been asked to complete this form on behalf of a student who is applying to volunteer with the Museum of Life and Science this summer.  It must be submitted no later than February 28 or it will not be considered.

The duties of this position include assisting adult staff in the supervision and instruction of groups of campers ages 4-13 in a variety of science-based activities.  The volunteer would be working with groups of up to 18 campers.  All camps are age-specific so the volunteer would only interact with one age group at a time.  Please complete the following information to the best of your ability.  All recommendations will be kept confidential.
